National Area Agencies on Aging
The National Association of Area Agencies on Aging (n4a) is the umbrella organization for the 655 area agencies on aging. Its fundamental mission is to provide services that make it possible for older individuals to remain in their homes.

Northwest Indiana Community Action
Local not-for-profit organization Area Agency on Aging and Community Action Corporation, as well as Northwest Indiana’s 2-1-1 Information and Assistance Call Center. Provides services to individuals and families so they can maintain independence and self-sufficiency. Serves Lake, Porter, Newton, Jasper, Starke, and Pulaski counties.

Alzheimer’s and Dementia Services of Northern Indiana
An independent not-for-profit organization serving eleven counties in northern Indiana, including Lake, Porter, La Porte, Elkhart, Jasper, Kosciusko, Marshall, Newton, Pulaski, St. Joseph, and Starke. Offers free programs and services to help people with Alzheimer’s or related dementia as well as their caregivers, families, and friends.

American Cancer Society

American Diabetes Association
Nation’s leading nonprofit health organization providing diabetes research, information, and advocacy.

American Heart Association
National voluntary health agency whose mission is to reduce disability and death from cardiovascular diseases and stroke.

American Lung Association
Fights lung disease in all its forms, with special emphasis on asthma, tobacco control, and environmental health.

National Multiple Sclerosis Society

National Parkinson Foundation
